
The Caring Collaborative, a benefit of membership in The Transition Network, fosters opportunities for members (women 50 and over) to remain healthy and energetic, and to support one another through short-term health situations. The Caring Collaborative is offered in four of TTN’s chapters – New York City, Long Island, San Francisco Bay Area and Philadelphia.

The Conversation Project “encourages you to make your end-of-life wishes known. As they say…start with “what matters to you, not what’s the matter with you.” Whether the conversation you want to have is about you or about a family member, there is a starter kit and videos to help you get it done.”

Patient Advocate Foundation is a national 501 (c)(3) non-profit organization which provides professional case management services to Americans with chronic, life threatening and debilitating illnesses. Patient Advocate Foundation’s Patient Services provides patients with arbitration, mediation and negotiation to settle issues with access to care, medical debt, and job retention related to their illness. They provide care management services, educational materials and live chat services at no charge to patients.

Provides tools, programs and resources to help patients and caregivers better prepare for their doctor’s visits and have better conversations with their doctors. Information is available in English and Spanish; free of charge.
